TRANSITIONAL HOUSING PROGRAM
Full-time and Part-time shifts available.
Locations in: Boonton, Chester, Dover, and Morristown.
Provide on-site supervision, counseling, and advocacy, supporting consumers in achieving goals outlined in their individualized rehabilitation plans.
Build skills in daily living using evidence-based practices, including:
-Personal hygiene and self-care
-Household chores, laundry, meal planning, shopping, and meal preparation
-Money management, budgeting, and banking
-Community involvement, meetings, and social engagement
-Recreational activities and interpersonal skills
-Accessing benefits and entitlements
-Medical case management, including appointments and prescriptions
-Employment and educational support
-Safe transportation (as needed) in agency vehicles
-Respond to crises and coordinate with appropriate personnel and authorities as needed.
-Provide medication supervision and track adherence according to agency guidelines.
-Collect service fees and maintain accurate documentation.
-Ensure safety, cleanliness, and compliance with agency standards, including fire drills, inspections, and reporting facility issues.
-Communicate observations to supervisors and provide thorough shift hand-offs.
Qualifications:
-Bachelor’s degree in mental health, social work, psychiatric rehabilitation, nursing, or a related discipline; a combination of one or more years of college plus work or life experience together equaling four years; OR a High School diploma and four years of related mental health experience.
-Must possess a valid driver’s license, with a good driving record.
